- Annual exams and screenings
- Birth control counseling and prescriptions
- Menopause and hormone therapy
- Fertility and preconception care

Do you offer telehealth visits?
Yes. Many appointments—such as follow-ups, medication management, and consultations—can be done virtually from home. Some visits, like annual exams and procedures, require an in-person appointment. Please note: Medicare no longer covers virtual visits; patients with Medicare insurance must be seen in person.
How do I schedule an appointment?
You can schedule by:
- Calling our office during business hours
- Sending a message through the patient portal
Our team will help you choose a convenient time and the right provider for your needs.
What should I bring to my first visit?
Please bring your insurance card and a photo ID. About 4 days before your visit, you’ll receive a questionnaire based on your appointment type. Completing this at home ahead of time will make your check-in smoother.
What insurance plans do you accept?
We work with most major insurance carriers. Coverage varies, so please check with your insurance company for eligibility, benefits, referral requirements, and in-network status before scheduling. Be sure to notify us if your insurance changes.
What is the “personalized care fee”? (Functional Medicine patients only)
This fee supports the additional time, services, and holistic care provided in functional medicine visits. It is not covered by insurance and is billed directly to patients either monthly (minimum 3 months) or annually.
What is your cancellation or no-show policy?
We require at least 24 hours’ notice for cancellations or rescheduling. Late cancellations or missed appointments may result in a fee. This helps us keep appointments available for other patients.
Do you offer menopause and hormone therapy consultations?
Yes. Our providers specialize in personalized treatment plans for perimenopause and menopause. Options may include lifestyle strategies, nutrition, hormone therapy, herbal medicine, and non-hormonal prescriptions when appropriate.
Do you provide birth control counseling and prescriptions?
Yes. We offer counseling to help you choose the best method for your needs. Options include:
- Pills, IUDs, implants, and injections
- Non-hormonal methods
- Fertility awareness counseling
- Female sterilization (when desired)
How do I request a prescription refill?
The fastest way is through your pharmacy, which will send us an electronic request. If we are not the original prescriber, please send a message through the portal to request that a Synergy provider assume your prescription or update your pharmacy information.
Where are you located and is parking available?
We are located in the Lovejoy Medical Building in NW Portland. Patient parking is available in the building’s garage (entrance on 25th Street). Take the elevator from the garage to the third floor—we’re in Suite 300.
How do I contact the clinic after hours?
Urgent concerns: Call our main number to be connected with the on-call provider.
- Emergencies: Always dial 911.
- Non-urgent issues: Send a message through the patient portal or email us, and we’ll respond the next business day.
Are you ready to commit to you?
Please check with your insurance carrier for specific eligibility, benefits, provider panel, referral requirements and/or in-network status for your specific plan before scheduling an appointment.
